چکیده

CARDIOVERSION has proved an effective and safe method for reverting ectopic arrhythmias to sinus rhythm.1'-" However, there is increasing evidence that capacitor electrical discharge may provoke arrhythmias in experimental animals12"-4 as well as in man.2' 6, 15 The majority of abnormal rhythms immediately following cardioversion are atrial in origin and are not serious. They generally represent one of three mechanisms: (1) a delayed "warm-up" of the sinus node manifested by sinus bradycardia, nodal rhythm, or escape beats, (2) increased atrial automaticity as demonstrated by individual or multiple atrial premature beats occurring either singly or in paroxysms of tachycardia, and (3) a defect in elaboration or conduction of the sinus impulse characterized by chaotic atrial activity with changing P wave contour and bradycardia interspersed with multiple and recurring ectopic beats with runs of atrial or nodal tachycardia. The first two mechanisms are transient and generally result in stable sinus rhythm within 1 to 5 minutes. The third mechanism almost invariably reverts to atrial flutter or fibrillation. The ventricular arrhythmias following cardioversion are less common but may be more serious. These are of two types: The first occurs immediately after the shock and involves development of ventricular fibrillation usually the result of improper synchronization. The second occurs after a delay of several or

Diphenylhydantoin prevention of arrhythmias in the digitalis-sensitized dog after direct-current cardioversion.

an average of 155 watt-seconds was required to produce ventricular tachycardia. Immediately after recovery from digitalis toxicity, an average of 23 watt-seconds produced ventricular tachycardia. The administration of diphenylhydantoin at this time immediately increased the threshold required for ventricular tachycardia to an average of 363 watt-seconds. In eight studies, diphenylhydantoin was ...

Cardioversion and digitalis drugs: changed threshold to electric shock in digitalized animals.

• Cardioversion is now widely accepted as the method of choice for terminating a number of atrial, nodal and ventricular arrhythmias." The technique consists of administering from a capacitor a specific underdamped monophasic electrical pulse across the intact chest. When the heart is exposed to electrical energy, there is a risk of producing ventricular fibrillation. With this new technique, s...

Quinidine for pharmacological cardioversion of atrial fibrillation: a retrospective analysis in 501 consecutive patients.

BACKGROUND Although quinidine has been used to terminate atrial fibrillation (AFib) for a long time, it has been recently classified to be used as a third-line-drug for cardioversion. However, these recommendations are based on a few small studies, and there are no data available of a larger modern patient population undergoing pharmacological cardioversion of AFib. Therefore, we evaluated the ...

Digitalis intoxication in patients with atrial fibrillation.

The characteristic arrhythmias induced by digitalis glycosides in patients with atrial fibrillation are illustrated, and their mechanisms are described. The two effects of the drug which are most important in the genesis of such disturbances are: production of A-V nodal block and arousal of subsidiary pacemakers. These properties account for slow ventricular responses, escape beats, and nonparo...
